Do you know a link on the web where the exact calculation formula for the SP500 index is given. Standard and Poors' site is not very helpful. I can think of lots of ways to calculate a "market value weighted index". One page on their site suggests that it is just the total market value of the companies. So if a company issues equity in a price neutral way the index would go up. That's not a "price index" in the normal sense as used in economics.

Hi James ..

Why wouldn't this same person just short the futures and use the proceeds to purchase the cash index?

This way they keep the premium and the rest is neutral.
