A similar thing happened to me with this same stock.
I put in a limit to buy at 16 when it was around 15.5.
Within a half hour, it took off to 16.5 and I didn't get
a confirmation, so I changed it to 17, again I looked
an hour later and it was at 17.5. I said I wouldn't
go higher than 18 for it, so I changed it one last
time. I still didn't get an confirms for about 4 hours
even though it stayed under 18. Finally I got one that
said I got it at 16 and that the other two were cancelled.
It really wasn't a goofup, the first order executed on time,
the report just didn't come through. I later
read a new warning that says not to change or cancel
orders during high volatility, because the reporting might
be delayed.

a day or so ago, i had an order in to buy NAVR at $15 - about an hour before the open, i cancelled it online when i saw it could be a bad day>>
