Infrared Imaging of the Breast: Initial Reappraisal Using High-Resolution Digital Technology in 100 Successive Cases of Stage I and II Breast Cancer. Our initial experience would suggest that, when done concomitantly with clinical exam and mammography, high-resolution digital infrared imaging can provide additional safe, practical, and objective information. Our initial reappraisal would also suggest that infrared imaging, based more on process than structural changes and requiring neither contact, compression, radiation nor venous access, can provide pertinent and practical complementary information to both clinical exam and mammography, our current primary basic detection modalities.

Thermographic imaging in the beating heart: a method for
coronary flow estimation based on a heat transfer model.

Intraoperative thermographic imaging in open-chest conditions can provide the surgeon with
important qualitative information regarding coronary flow by utilizing heat transfer analysis following
injection of cold saline into the aortic root. The heat transfer model is based on the assumption that
the epicardial temperature changes are mainly due to convection of heat by the blood flow, which
may, therefore, be estimated by measuring the temperature variations. Hearts of eight dogs were
exposed and imaged by a thermographic camera. Flow in the left arterial descending (LAD)
coronary branch was measured by a transit-time flowmeter. 20 ml of cold saline were injected into
the aortic root (just after the aortic valve) and the epicardial temperature images were recorded at
end-diastole, for 20-30 s. Different flow rates were achieved by 1 min occlusion of the LAD, which affected a reactive hyperemic response. The dynamics of the temperature in the arterial coronary tree was obtained by averaging the temperature over an edge-detected arterial segment for each frame. The heat transfer equation was curve-fitted, and the flow-dependent heat transfer index was correlated with the experimentally determined coronary flow (r = 0.69, p < 0.001). In summary: a method for quantitative estimation of coronary blood flow by thermography and heat transfer analysis was developed and tested in animal experiments. This method can provide important information regarding coronary blood flow during open-chest surgical procedures.

To carry out this mandate, the Center has recruited a multi-disciplinary team of health professionals. Their ability to provide a number of specific and related health-oriented services in a community environment is facilitated by prompt access to radiology and laboratory technology and help from support personnel, selected for their capacity to appreciate our patients' health and personal needs.

The following services are currently offered:

1. The VILLE MARIE BREAST CENTER brings together a multi-disciplinary team of professionals experienced in the diagnosis and management of breast pathology. This team is composed of technicians, radiologists, physicians, surgeons, oncologists, pathologists, plastic surgeons and psychologists so as to provide a prompt diagnosis by combining breast imaging (mammography, infrared, ultrasound, etc), clinical and cytology evaluation. Fine needle, core or open biopsy, including ultrasound digital mammography and stereotaxic-guided, will be carried out, if needed, prior to treatment planning. For more details click here.

2. The VILLE MARIE OBSTETRICS, GYNECOLOGY AND MENOPAUSE SERVICE provides access to evaluation and management of obstetrical, gynecological and menopause-related issues for those patients who are not currently followed by either a family physician or a gynecologist.

3. The VILLE MARIE OSTEOPOROSIS CLINIC, run jointly by both rheumatology and endocrinology specialists, ensures access to information, diagnosis, prevention and treatment of osteoporosis.

4. The VILLE MARIE THYROID CLINIC provides joint imaging (ultrasound), endocrine, surgical and pathology expertise in the evaluation and management of thyroid and parathyroid-related disorders. For more details click here.

5. The VILLE MARIE PLASTIC AND RECONSTRUCTIVE CLINIC provides detailed information and access to current options in modern plastic and reconstruction techniques.

6. The VILLE MARIE ONCOLOGY CENTER consists of a multi-disciplinary team of surgical, radiation, and medical oncologists, palliative physicians and psychologists to improve coordination and access to evaluation, diagnosis, treatment and follow-up for cancer patients and to ensure adequate support for both patients and their families. 7. The VILLE MARIE OTOLARYNGOLOGY/HEAD AND NECK SURGERY CLINIC is concerned with the evaluation and treatment of diseases of the oral cavity, the larynx, the neck and other areas of the upper aero-digestive tracts.

8. The VILLE MARIE GENERAL SURGERY CLINIC is primarily involved with the diagnosis and management of abdominal and gastrointestinal-related disorders.

9. The VILLE MARIE PSYCHOLOGY SERVICE is available to help patients cope successfully with the ongoing stress and emotional difficulties that their illness may cause.

10. The VILLE MARIE ALTERNATIVE THERAPY SERVICE was initiated in response to patients requesting access to interaction between traditional and alternative medicine.

11. The VILLE MARIE RADIOLOGY CENTER, designated as a Screening Center, uses accredited (Canadian Association of Radiologists) mammography imaging. In addition, radiology imaging and high-resolution ultrasound provide for further evaluation of head and neck, chest, skeletal, abdominal, pelvic and obstetrical areas.

12. The VILLE MARIE VOICE AND LARYNGEAL ENDOSCOPY CLINIC (planned) will combine expertise in laryngology and speech pathology with currently available endoscopy technology to promote the prevention, diagnosis and management of voice-related disorders and laryngeal tumors.

Recognizing the importance of patient information, the staff of the Ville Marie Medical and Women's Health Center organizes a yearly programme of complimentary support and information sessions for patients, their families and friends (see Patient Info. and Support above).

Teaching and Clinical Research
In view of the community shift in patient care, the Center assumes the responsibility of providing medical and clinical education, under direct supervision, for McGill University medical students and residents as well as other interested health professionals. A new computerized medical data information system has been introduced to promote future quality control and clinical research and to improve comminucation with referring physicians. All clinical trials carried out by the Ville Marie Oncology Research Center are evaluated and conducted under the supervision of the Ville Marie Medical Ethics and Review Board.
