SL, ALL off label usage of medicine and efficacy is worked out between clinician and patient. I am still troubled by these dangerous clinicians that let their patients get so out of hand they have to carry in their meds in a brown paper bag! I know you have some excuses for it but it is still bad medicine and spawns from suspect true clinician patient relationship. I think you will be surprised how clinicians will "suddenly" get interested in research if the financial incentives were there, once again free market society will work in that instance. And as you know most everyday medicine is prescribed off label, so why do you need FDA approval except for safety in the vast majority of how people are prescribed medications? How many clinicians will survive in free market society if they practice medicine that has no efficacy? Only ones I can think of is HIRED clinicians who have no incentive but to collect a paycheck. One thing you can take to the bank SL, big brother social answers will never work no matter how well meant and that includes the FDA being in the efficacy business. I will grant you, though, that I would be willing to let the FDA have a minor role in efficacy but certainly take it out of the approval process equation. ANY clinician worth his or her salt can take a few minutes of EVERY day to review required efficacy studies and decide for themselves along with their patients. PLEASE SL, EMPOWER THE PEOPLE! You will never be disappointed in having faith and respect for the American general public. They certainly will do no worse than BIG BROTHER.
