It depends upon whose "need" you are referring to. If you are "in need" of help, and a friend shows up, you are fortunate. The friend who appears at the time of your need is a friend indeed.

It means that if when you are in need, someone behaves toward you as a friend (ie helps you), that person is a friend indeed.

A friend in [your] need, is a friend indeed.
