Don; I thought my 1099R from Schwab was wrong, but after talking with people in their tax dept. turned out it was correct. I did a Roth conversion, recharacterized that transaction, and re-converted to a Roth again, to get a lower tax. One 1099R is the sum of the recharacterization and the conversion. The recharacterization will also be reflected on a 5498 on your April statement. If you did something similar you will get additional 1099R's, the distribution code in box 7 tells the IRS what they're for.
