All bids and asks are indeed placed on public display save for orders containing restrictions. In the latter case, the order will never show up on Market Depth (equivalent of Level 2). It acts as a phantom order until it is triggered.

Unless the stock is trading on the CDN exchange (equivalent of OTC-BB), you do not deal with Market Makers. However, some brokers play the spreads in certain stocks.

As to commissions, the only reason why they are higher in Canada is the lack of competition. A round trip actually costs them $9. That's $4.50 per trade. They are making a killing on commissions!
