I have it. He could have a minor surgical procedure in which he either had a wedge of skin removed, to bring the double l's together at the bottom, as you suggest, to create "Vivian," or he could have a small skin transplant to bisect the V, and separate its two parts, making it an L, and his tattoo 'Lillian.' Then he could advertise in the personals for a girl friend of the appropriate name. Or he could advertise first, and let the responses determine which instructions he gave to the surgeon.
