Spreck, someone else can answer for athm, but I will answer for Roadrunner (Time Warner).

When I signed on, I was an early adopter. They waived the install, (normally $40 or so).

Modem rental is included in the base monthly charge of $39.95.

They installed a network card in my PC which is mine to keep if I cancel, but the modem belongs to them.

I added a second IP address for the second computer in our house, and each additional IP address is $10 per month.

Install fee is set by & kept by ATHM's various cable partners.
Mine, w/ TCI was $75, "50% off" the $150 bloated, hopefully
never charged, full install price. Two guys at my house
for over an hour, checking cable signal strength, installing
new cable jack if needed (next to computer), installing Sw,
configuring, testing, etc.

They need to roll out the retail self-install version ASAP.
(to lower the cost for those who don't need help).

$5 of my $40 monthly fee covers modem rental.

Install was super easy for me, didn't do a thing. Lots of people may
actually like that, I've helped friends get hooked to ISP's
and those friends (non-techies) got quite frustrated w/ lack
of help from those dial-up ISPs - this "We'll come to your
house and do it for you" may find a good audience...
