None of the patients in that study had success with MUSE alone or Viagra alone but they succeeded using both.

EFFECTIVENESS OF COMBINATION THERAPY OF MUSE AND VIAGRA IN THE SALVAGE Of ERECTILE DYSFUNCTION PATIENTS DESIRING NONINVASIVE THERAPY

INTRODUCTION AND OBJECTIVES:The advent of noninvasive pharmacotherapeutics and the potential for some patients to fail one regimen of treatment prompted us to investigate combination therapy of two different medications. The objective of the study was to determine if Viagra (an oral PDE-5 selective inhibitor) in combination with MUSE (intraurethral PGE1) was more efficacious than either of the medications alone.

METHODS: 16 patients (11 post radical prostatectomy and 5 with a diagnosis of organic erectile dysfunction) mean age 56 years (51-66) were included in this study. A detailed history and physical were taken and vascular risk factors noted. Patients had failed either 100 mg Viagra and/or 1000ug MUSE I.U. None of these patients desired intracavernosal injection, and hemodynamic testing was obtained in only 6/16 patients. Combination therapy was initiated using 100mg Viagra orally 60 minutes prior to intercourse and 500 ug of MUSE intraurethrally immediately before intercourse.

RESULTS: In all patients, 100mg Viagra or 1000ug of MUSE independently was not efficacious in inducing an erection sufficient for penetration. These highly anxious self-motivated patients were then converted to a combined therapy. Combination therapy including a lower doses of MUSE (500ug) was most effective in inducing an erection sufficient for penetration. At three months, all patients are at present reporting excellent results.

CONCLUSIONS: Combination therapy of MUSE and Viagra may be more effective in the salvage of patients who desire noninvasive therapy but fail single treatment modalities. This may be due in part to high doses of PGE1 inducing contraction in the corpus cavernosum via thromboxane receptors or aggravation of the norepinephrine-induced alpha adrenergic mechanisms in the penis.

DaiS, it must be noted that combination therapy is outside the approved labeling of both Viagra and MUSE.
