John,

Thanks for updated index. LOL - You must have been reading my mind. I was in your profile checking out embedded systems indexes this am:-))

INSTITUTION OF ELECTRICAL ENGINEERS
Processing industries (G2.17.8)
One company reported having found that of its embedded systems
- 25% had date functionality
- 25% of these were non-compliant
- 6% were complex supervisory systems

The 25% non-compliance figure was confirmed by another report. There was also agreement that this figure is typical of those reported at conferences and in private conversations. It was also noted that those reporting a 25% failure rate have evidence of a sufficient number of specific individual failures to explain that number.

SOME EXAMPLES - Partial List below
iee.org.uk
Effluent control system
- Continuous discharge instead of shutting down after an hour
Hydrogen sulphide analyser
- Failure to detect
Sand monitoring equipment
- Failure, with possible serious consequences for pipes
Compression monitors
- Complete failure

I was surprised how high this figure was for process control. Their site breaks down avg occurences for various industries.
iee.org.uk

They've added a lot to their site since I was there last year.
