This form cannot be dismissed as just "paperwork". On the final page the market maker must certify that he has a reasonable basis for believing that the information is accurate in all material respects and that the sources of information are reliable. This usually means audited statements.

By the way, only one market maker is required to submit this form. Once it is submitted other MM's can "piggy-back" on the initial submission.

The form is not submitted to the SEC. It is sent to the OTC compliance unit of the NASD a minimum of 3 days (I think) before the MM can post bid/ask quotations.
