'Foreseeable incidents offshore

The Y2k Problem may cause the following incidents on or near an offshore installation:

Loss of process control: Most control rooms are provided with all-computer process control systems. Failure of such a system could result in uncontrolled operation of the plant with a multitude of potentially major hazardous outcomes.

TR impairment: Integrity of the Temporary Refuge on an offshore installation is maintained by fire / blast walls and artificial internal atmospheric over-pressurisation to prevent the ingress of gas or smoke. Failure of the Heating, Ventilation and Air Conditioning (HVAC) System would result in TR impairment and shut down of oil production, and de-manning of the installation if the problem persists.

Loss of Power Management: Power management systems are provided on offshore installations to secure electrical power supplies to essential life support systems when power generation systems fail and during emergencies. Failure to keep safety systems energised could result in an inability to detect hazardous situations and/or protect personnel against the effects of fire and explosion.

Communication failure: If the primary satellite communication links between a platform and the land station, or the telephone exchange equipment in the company's office fails, then communication from / to the platform is limited to back-up systems, i.e. local VHF radio and MF/HF radio links. Lord Cullen concluded that the failure of communication systems contributed to the outcome of events during the Piper Alpha disaster (Cullen 1990).
