You are swearing under penalties of perjury that you answered the NASD portion correctly (ex. you are not going to disseminate the info to the public). The top portion of the form is Tradescape's info and is not part of the penalty of perjury info. If you happen to make a simple mistake in entering you social security number there is no problem. If you end up trading with them you should correct your mistake.
