Re : If you contact them [the CBOE] and ask, they'll set them [new strike price option series] up for trade usually within a day. Jon Koplik has done this often.

Important footnote to this -- the options marketmakers have to agree to do this. They would rather keep their lives simpler (by NOT adding zillions of new options), if they feel that the existing options provide a decent order flow to allow them to earn a living.
