Hi Keith,
The transfer agent for ADOT is Oxford Transfer in Portland, Oregon.....(503) 225 0375.
However, all previous attempts to get information of any kind (to my knowledge) have failed. Oxford has informed me and several others that have called that they need written permission from ADOT to release any information.
Of course ADOT COULD, at some time change this.
