Subject: F.A.S.T system.

Alain-
The F.A.S.T. system (I can't remember what the initials stand for, and
somebody stole my flyer) is a rapid access intraosseus system designed
to be used in adults. The system implants a steel tipped, flexible
catheter in the manubrium, and according to the papers, goes from
prepping the skin to infusion in about 70 seconds, with flow rates of up
to 250 cc/min with a syringe. A Canadian company, Pyng Medical, makes
the product (check out www.pyng.com) which is FDA approved, but not in
widespread production yet. Its designed to be used in the field (even
combat) by even non-physician providers. If their numbers hold up, I
WANT IT. I'm sick of messing around with central line kits in a code,
when I could be infusing in a minute and a half.

Looks neat. Has anyone used it? It shouldn't get in the way of CPR, and
it can be inserted in seconds. Is this the one that was developed/tested
by the Israeli army?

There is more information at www.pyng.com, a poster by Dave Robinson at the
last ACEP Research Forum, and a paper by Lark Susak submitted to Lancet
describing the first 60 uses. It's approved by the FDA but not marketed
yet, available as a beta test while Pyng perfects the device and arranges
for mass production.

It's not the bone infusion gun developed by Wais Med Ltd PO box 2982 Even
Yehuda 40500 Israel (Waisman M, Waisman D: Bone marrow infusion in adults.
J Trauma 1997;42:288-293). The B.I.G. is a spring-loaded device that goes
in the tibia and comes in adult and pediatric sizes. You dial in the
desired depth beforehand.

The F.A.S.T.1 comes only in adult size and requires the user to press hard,
with a safety catch that gives way and prevents it going too deep. It
includes attachments to secure the tubing which should work well in EMS. I
have used the F.A.S.T.1 in several patients and can endorse what the flyer
says.

Keith
The F.A.S.T. 1 is easy to place. We've had them in our department for about a year. I don't think we've had the need to do CPR with one in situ yet but it comes in very handy when the patient needs fluid resuscitation and has no access.
BTW Pyng Medical is Canadian.
