Have you read Tom Wolfe on the effects of speech patterns on social standing? As usual, a treatment at once humorous and elegant... The biggest problem is that it is very difficult to cure deeply ingrained speech patterns (look at Susan Estrich), and so many people think that they have toned up their speech when they have many tell- tale signs remaining, including signs of excess effort. It is actually, ultimately, a somewhat painful subject, since many otherwise capable people can be undermined by such things...
