I think that the Iranian people preferred the Ayatollah to the Shah, and put him in charge.

Again, history. The Iranians put in a secular government under Mossadegh, 1953 I believe. We didn't like his politics and thought him insufficiently responsive to our geopolitical interests, so we bounced him out and put in the shah. The shah stifled all means of legitimate political dissent, but couldn't stifle religion. So political dissent - inevitable in a horribly governed country - flowed through religion. Result: medieval theocracy. Unintended consequences indeed.
