The Space Shuttle would have to be the #1 most reliable.

Ariane 4 is 81/84 since 1988, or 40/41 since the newer versions started in 1994. The Atlas 2 has been 100%, but doesn't have enough uuumph for big sats. The Proton variations all are in the low 90% range. That's about it. Sea Launch is 100%, as it currently stands, and can lift more than all the others into GTO. Ariane V ought to come up to the same reliability as the Ariane IV. They did several TEST flights with it. It will have the greatest lift power. It can do 18,000 to 20,000 kg to LEO---that's 40 G* sats! Don't tell Maurice that!
