Medical Humor: The Patient Was Alert And Unresponsive January 04, 2000 Medical records often contain humorous mistakes and I recently shared a list of such snafus with you. Here are some more examples culled from records from various major hospitals. One of my favorites is: Patient alert and unresponsive. Now that would be difficult, but the sentence is obviously the result of a typo. Other funny entries are simply the result of poor sentence structure. Here are some more that are pretty much self-explanatory: --The baby was delivered, the cord clamped and cut and handed to the pediatrician, who breathed and cried immediately. --The patient lives at home with his mother, father and pet turtle, who is presently enrolled in day care three times a week. (Wow, that’s one smart turtle!) --Bleeding started in the rectal area and continued all the way to Los Angeles. --Occasional, constant, infrequent headaches. --Examination reveals a well-developed male lying in bed with his family in no distress. --When she fainted, her eyes rolled around the room.
