They are not incompatible. Several years ago either China or Korea (or both, can't remember) developed a CDMA SIM specification, but have not yet deployed it.

When the UWCC allied with the GSM Association they set about developing a TDMA specific SIM specification that builds on the GSM specification. Work on this began mid summer 1999, and I assume it is reasonably complete.

The architecture of a SIM (or any IC smart card consists of a microprocessor, an OS masked at the the silicon FAB, some RAM for calculations and EEPROM where directories and files including executables are stored). TDMA or CDMA specific functionality can be added to EEPROM which is quicker than remasking an OS. AMPS, TDMA, and CDMA in the US use essentially the same authenticating engine with the CAVE algorithm. This could be added in EEPROM on a SIM chip with a GSM specific OS. This is the approach that was taken when the first GSM/AMPS handsets and SIMs were delivered. in early 1998. Eventually an OS could be coded and masked with most functionality on the OS freeing space in EEPROM for additional applications.
