OT--TA One step in containing costs is to respect the Advance Medical Directive of patients under the Patient Self-Determination Act of 1990. One study indicted 80% of all costs under Medicare occur in the last two weeks of life when it is completely pointless. People are more than willing to self-limit (decline extreme measures) at or near end of life. They are being ignored. They are receiving expense, pointless medical care without their consent and against their previously expressed wishes...prolonging their suffering and their families' suffering.
