Possibly Northern Light could be the search engine you are looking for. After 200,000 or more sites that contain the words you are looking for it categorizes them and puts them in different folders. For example: Robert Lichello comes up with a financial folder and a medical folder. This covers both the Doctor as well as Mr. L., but lets you concentrate on the one of main interest.
