The 'O' rings are the classic management failure. the engineers were screaming to stop the launch and were overridden by their senior management.

I was thinking about the Challenger last night....my sister worked briefly for the same law firm Christa McAuliffe's widower Steve works for. She said he was a competent lawyer, not ambitious, just an ordinary guy who wanted to work and raise his family in an ordinary way, which would have been the course of his life had his wife not been chosen for the Teacher in Space project. After the tragedy he focused on his kids and his work (in that order) and stayed out of the limelight. I wonder if he contacted any of the WTC families to offer support and sympathy...if anyone knows how it feels to have your life blown apart in 10 seconds he does.
