I realize how it has to work, matter of fact I think I saw a very nice description of the technology somewhere not long ago.

The way I'm reading this your LEC (usually an RBOC) must have Telstra's SS7 switches in your CO, therefore it would involve "messing with their equipment" to provide something that takes away from their revenues.

"Telstra enhanced its SS7 Network Signalling System to allow it to communicate directly with NetSpeak's core IP telephony communications products, which includes it WGX telephony gateway, connection and directory services and network management services. This modification allows regular telephone calls to be placed over IP networks such as the Internet and SS7 networks, to 'see' NetSpeak's WebPhone IP Telephony software as if it was a regular telephone."

It's also utilizing voice over regular IP, the quality of which is still the subject of hundreds of stories.
