I don't think they have to mess with the RBOCs telephony equipment that much. If they have an SS7 interface the call can be directly re-routed to the ISP server and the server can do the switching for you. All they have to do is allocate enough bandwidth over your POTS line for the voice which will be compressed. Outgoing call will handled similary.
I assume however that some sort of arangement would have to be made between the ISP provider and RBOC to bill you for outgoing calls. This could be a cozier arrangement than you think. Remember a RBOC doesn't get paid for signals that terminate in a busy signal. If the ISP provider is running SS7 then he should be capable of handling billing also. In effect the ISP provider could become a little RBOC of its own.
