One big diff is that the Prez of Iran doesn't have much actual power... The President of the USA does. The Supreme Leader is much more Powerful than the Pres of Iran and far more Powerful in Iran than the Pres is in the USA. People outside of Iran take Amadenajad far more serious that the people within Iran.

The Assembly of Experts elects and dismisses the Supreme Leader on the basis of qualifications and popular esteem--none have ever been dismissed. The Assembly of Experts is responsible for supervising the Supreme Leader in the performance of legal duties."
