Those problems are not confined to voice recognition software.
It happens with live secretaries and tapes all the time.

I remember receiving a letter telling me to proceed with my recommended motion for "some rejudgment."

The motion I recommended and presumably the one I was being authorized to file is called a motion for "summary judgment."
