Isn't there always a lot of blood and wouldn't the hole be deep into the cavity of the skull?

Needless to say I'm not a ballistics or a gunshot-injury expert, but I believe that if you were shot at close range with a .45 the entry wound might be a smallish hole, but the exit wound would be a real mess.
