jhild,In addition to your post I received several PMs asking me to confirm whether or not the posted Form D matches the one I received from the SEC. (Jeez, thanks for vote of confidence, folks.) I am not going to absolutely swear my life away on this, but....

Everything posted looks like an exact duplicate of what I received. Exhibits B, C and D are not included on geocites, but they are boiler plate forms, not specific to PABN. Some of the financial info is difficult to read on my copies too. I suspect Disclosure either copied from microfilm (heaven forbid!), or if the documents were filed electronically then the originals had been reduced to fit the format requirements.

PCModem, I paid about $16 for my copy (I received 61 pages). I often forget that some SEC documents are not indexed or available on-line. The email to the SEC works well, PublicInfo@sec.gov They usually answer within 24 hours; they give you an "HO[number]" and then they forward this to Disclosure, Inc. Then you have to call Disclosure to tell them to send it. It's an 800 number. In the past I've tried to order directly from Disclosure and it doesn't work that way.

BTW, I happen to know that at least two other requests were made for this document before I sent my request in because the Disclosure clerk was confused about why I had "so many email addresses"...
