The only ISP I know of so far that is COMPLETELY FREE and has broad USA coverage is netzero.com and there are two SI threads which discuss it and related themes:

Yes, I do use NetGenie as one of my ISPs, and the have instituted
an AdBar as a requirement for using the service. I bought in
before this was part of the service, and was disappointed to
see this addition. As an ISP they are "OK", and use GNN as
dialup provider in most locations. They have web-based email
only, and also allow you to view Usenet news via the web.
Since the one-time payment allows unlimited use for life
(the life of the company, anyway...) I still am not unhappy
with the deal.
