That seems to be an excellent idea Jack. What we would likely need: Full Name -- Number of shares -- Province or state -- Signature If people are hesitant to give out their names or the number of shares controlled, directly or indirectly, it would still be a good idea to give their alias' and if possible the number of shares owned. It would be a good idea to include the number of shares held directly and controlled indirectly such as in your case Jack. That would give us a better idea of our position. Kind Regards AK
