There are these references as well:
This page is the starting point for the USPTO's free trademark database, which includes the full bibliographic text of pending and registered trademarks. It was last updated to include data from the PTO's internal trademark database as of September 1, 1999. This means that marks registered and published in the Official Gazette -- Trademarks through that date should be included. Pending trademark applications are usually entered in the PTO database one to two months after filing; thus, pending trademark data for the preceding two months are incomplete. The next data extraction is expected during the first week of November 1999; that data should appear in this database some time later that month.

About four months after filing, an examining attorney at the PTO reviews the application and determines whether the mark may be registered.

One can reasonably conclude that the application, even if filed, has not been registered at this time and the use of the R is not yet appropriate.

Since the text updates will be entered in the next week, we will know definitively whether or not CodeCare has even been applied yet quite shortly.

Btw, looks like MedsAmerica has a hammerlock on winning any dissent to an application for the word mark "CodeCare".
