I have a fees example that I downloaded from Cyber's WWW site a month or two ago; it may or may not be currently accurate, but on a 3000 share order with multiple fills it shows the $0.0035 fee applied to 1600 shares:

1200 Island direct, "additional processing fee" (APF) = 0;
1200 Attain direct, APF = $3.50 ("1000 share maximum")
500 REDI via SNET, APF = $1.75
100 INCA via SNET, APF = $0.35

The best I can do in inducing an algorithm from this is: the APF applies on orders over 1000 shares to each partial fill subsequent to the first, and with a maximum charge of $3.50 per such partial fill.
