The network operators themselves purchase cards directly from smart card manufacturers, Gemplus, Schlumberger, Bull, Oberthur, Orga.

These manufacturers supply representation to the principle standards body (SMG9 a part of ETSI), to SCAG (the Smart Card Action Group of ETSI) and to the regional interest groups of GSM like GSMNA in North America to evolve specifications, and work closely with the handset vendors.

The new development I spoke of is that 3GPP2 (cdma2000 oriented) has just transferred responsibility of UIM specification development to SMG9 for all ITU2000 UIM specification development. This indicates that there will in fact be a UIM in all 3G flavors. In addition this development could work its way backwards to 2.5G or even 2G cdma wireless where it would probably be an option as opposed to a requisite in 3G.

Like it or lump it GSM is way ahead of CDMA in terms of application development for wireless data. The SIM (or UIM is a great facilitator of this).
