When was the first 200mm fab? Wasn't it IBM in 91, and didn't they have a lot of problems?

Was it not until the 1994 time frame that a bunch of 200mm fabs started kicking in?

Are their any full blown 300mm fabs out there or are they still just pilot lines?

It would seem another sign of a top would be when the next generation wafer sizes start kicking in. It would seem very hard to judge the output it will create.
