More on the medical group suit,

The Plaintiff seeks actual or compensatory damages in the amount of $3,030,000 plus $5,000,000 in punitives.

ProVista HCA, located at the PABN headquarters in Hauppauge, is alleged to be the predecessor of PanAmerican Healthcorp, with Ray Gamble being the chief of staff.

I did a quick check for any business or corporate records under ProVista, including archive records, but no luck. Looks to be yet another low-profile enterprise.
