but from here we go on, question is - What next.

Castro must go. For someone to rule any country 40+ years seems so outdated to the point of absurd. That communist health system (at least the version provided to him) seems to be doing him good so I suspect he'll be alive for many years to come. Maybe some third country could provide him sanctuary (Spain?).

For the people of Cuba, they have several models of throwing off communism to choose from. (Poland, Romania, Russia, etc.). Maybe they'll be original and come up with a new way.
