There is some data popping up pointing in the direction of higher health care costs due to lifestyle factors you suggest that were not as prevalent even 20 years ago. Much of the the increased 'productivity' may turn out to have unmeasured costs after all.

If hedonic pricing was applicable to health care, you should be able to point to reduced expenditures in the health care basket.

Huh!? How about rising expenditure but faster rising benefits?
