People don't expect to have accidents, and have little warning before they do. So the premiums charged pretty fairly reflects the expected return. There is also the fact that many cars are financed, and the financing companies require that the cars be insured (so there is a very large pool of "healthy" applicants).

People don't expect to have heart disease or cancer, either, but generally have an idea something is wrong in plenty of time to apply for an insurance policy.
