Whenever I hear stuff like this I'm so glad I work for myself. When I screw up, I still have to answer to my client, which is hell because there are no small screw ups in photography. There is something to be said for being able to come clean and say to them, "I screwed up here, what can I do to make it up to you?" instead of trying to put the blame somewhere else.

You have to be paranoid to survive in any biz. You always have to check and recheck, have back ups for every system. If you think you are going to be able to run a business that never has problems you are in for a rude awakening. The best you can do is have a really good system for resolving them. Nobody loses customers because they have problems, they lose customers when they don't deal with the problems promptly. In order for you to deal with issues promptly you have to acknowledge the problem is yours in the first place.

The worst thing companies can do is to punish those that come forward and point out basic problems. What they should do is reward people for finding something wrong. The end of every memo should be, "Tell me what is wrong with this scenario."

Yeah....right
