Just some. Ex.: Young person (cheaper wage rate with less than typical education versus the older person who has been replaced) working with any type of equipment. Been instructed by expedient managerial process to highball the machine (insert favorite machine here) and the procedure of manufacture of the goods. Forgets to oil/lubricate the machine-works; or lives more vicarious lifestyle resulting in stress related mistake: End result is more widgets at higher cost. Seen this happen all too often?

Only takes one big mistake to ruin productivity. It is as good as the operator of the process. How many times you see some software not work out as planned? Planned productivity increase is only attained through adequate managerial process and training for implementation of method, etc.
