Eric, interesting analysis, thanks. As I read these arguments I can't help but ask a rather obvious, but infequently presented, question:

Is there a rationale solution that specifies a level of usage and perhaps the effects of an eventual (so-called) killer-application that could satisfy both the low cost expectations of users for bandwidth and applications, against the revenue requirements of the providers?

Consider: the investments that were made by service providers for spectrum, and future capex for all other forms of infrastructure and carrying costs.

Would anyone care to venture an algorithm or formula that demonstrates these relationships leading to breakeven, or dare I suggest, a profit?
