I like the idea that Iridium had of a terrestrial module that plugs into a larger satellite handset when necessary.

You could keep the satellite module in your car while using the cute module while around downtown. The terrestrial module would contain the subscriber identity module that would be the same for the satellite handset, thus providing transparent service to the subscriber.

I don't know about sat to terrestrial handoffs but cell reselects (that would happen when your phone is powered on but not in call mode) are very possible with very little but a bit of extra software required on the user terminal end.
