You are quite right, the space- based phase is still a gleam in an engineers eye. That is the ultimate aspiration. Right now, defenses are likeliest to come from the ground or the sea. If they are deployed in allied territory, or launched from ships, the possibility of catching the rocket in a boost phase still exists.

I am not well- enough acquainted with the orbital transport issue to make an intelligent comment.

The "keg of nails" threat is interesting, and there regardless of ballistic defense issues, as you note. I would venture that that is a primary contingency that military R&D would like to address.......
