It isn't unusual for government agencies to have trouble with their bookeeping. The IRS has had a big outage for decades now. The biggest problem seems to be that the employees don't take record keeping seriously.

No, it no doubt goes back 20 years, at least. A growing percentage.

Not only does it reduce military and economic strength, it also increases the "moral hazard", where defense contractors, consultants, writers, politicians, foreign lobbyists and other thieves feel they have a green light to steal from the Treasury.

It establishes a lax "who cares?" attitude for other departments and reduces public confidence in their government.
