On December 7, 1941 the US army had a total of 158 tanks, most of them obsolete models desiged ten years before. So they hadn't produced any tanks? Sure they did, but they sent them to the British and the Soviets as fast as they could roll off the lines. When they finally got a significant army onto a foreign battlefield-almost an entire year-later the tanks were so bad they were shot up like lame ducks by the enemy. They finally won the war-by sending so many men and poor-quality tanks (and other, better assets, including nuclear weapons) overseas that the enemy was totally overwhelmed.

What they didn't do was stop the war while a bunch of stuffed-shirts with an agenda held an "investigation" into "what went wrong".

I'd also cite the problems of the Union Army in 1862 and 1863, or the American blank-eyed mob in France in 1917, but even you can get the point...
