I'd guess it works in the scenario you described, because the patient has potentially nothing to lose.

How about this one : "single person, recently unemployed, no COBRA, no source of income, but has a house thats getting paid for, and no other health insurance".

A friend of mine, was recently refused treatment for brain tumor, because she did not have $100K to put up front, and had no insurance.

IMO, 'the people in the middle get shafted'.
