We had a discussion on that on the PRR thread a few months ago. My feeling is that accommodations should be made, but only up to a point. If the flight is full and nothing can be done, and they are over some predefined measurement, then discreetly they should be asked to (a) wait for a later flight where an adjacent seat is left empty; or (b) asked to buy an adjacent seat (and have someone volunteer for denied boarding). It would also help it the airlines configured the cabin so one row, perhaps in the back, had two seats instead of three....regular sized seats, but not all the way across, so they would be available for this situation. There's no good answer though.
